Transition Conference
The goal of the 2011 conference is to increase understanding of how effective
transition planning addresses all aspects of adult life for people with
disabilities.
Workshop Sessions:
Presenters have submitted proposals for presentations that address one of
the following objectives:
- Research based or evidence based practices or strategies related
to one of the conference strands.
- Provide youth, families and professionals with the knowledge, skills
and ability to assist in making transition to adulthood in a manner
that 1) maximizes lifelong functioning and potential and 2) overcomes
challenges and barriers.
- Highlight lessons learned, present models for successful student-directed,
person-centered transition planning and service delivery, educate and
promote the importance of the transition process and establish high
expectations to enable each youth to be contributing members of society.
- Provide systemic solutions to expand the possibilities of ensuring
all young adults have opportunities to learn and develop skills, participate
in community life, and live and work in a setting of their choosing.
The conference will include interactive workshop presentations that support the
conference goal, in the following areas.
Heighten awareness of community services available to support youth
and their families with planning for and achieving goals related to
independent living, community involvement/integration, social and recreational
opportunities.
Raise expectations and inform consumers and professionals on realistic
opportunities for competitive employment, employment resources, and
employment accommodations. Understand the responsibilities of all stakeholders
in the area of employment options.
Provide strategies and examples of replicable successful transition
services that promote education at the secondary level and continued
development beyond secondary education. Increase understanding of secondary
transition requirements, how to write transition plans within IEPs and
504 plans, including forms and legal aspects. Increase awareness about
post-secondary education experiences and programs from the application
process to graduation/completion.
Describe ways to collaborate with health care providers and community partners to improve quality of health care transition; describe ways to prepare youth with special health care needs to transition to the adult health care system; and demonstrate how to include health goals in IEPs and transition plans.

Increase understanding of the support needs for youth and young adults with mental health disorders in all areas: community living, employment, educational and health care.

Speaker Registration and Travel:
- As a benefit to approved speakers, a one day conference registration
(for the day of presentation) will be complimentary. However all speakers
and presenters are responsible for their own travel and lodging expenses.
All speakers will be required to register online under the applicable
registration category.
- Presenters interested in attending additional conference days will
need to register and submit the registration fee.
- Consumers and family members who are presenting: there are some scholarships/financial
assistance available to assist with registration fees, travel expenses

Selection Criteria:
- All sessions will last 75 minutes. The following criteria will be
used in selecting proposals:
- Interactive format which allows participants to demonstrate skills
or implement knowledge learned
- Participation of youth, and/or adults with disabilities as presenters
- Potential for practical application and to attract and stimulate attendees
- Inclusion of innovative ideas or concepts, which demonstrate originality
and creativity in content and format
- Clarity, organization, feasibility, and degree to which the proposal
addresses the purpose of the conference
Presenters Calendar
- Proposal Due Date: March 31st
- Speakers Notified: June 2011
- Speaker Registration: by September 17th
- Final Presentation: due September 24th
- Presentations: to be given October 23rd -25th
