Registration Information

UPDATE: Early Registration deadline extended to October 2, 2009

Conference participants are strongly encouraged to register using the Illinois Transition Conference online registration and payment system managed by Complete Conference Coordinators, Inc.

If unable to register online, the 2009 Illinois Statewide Transition Conference Registration Form can be used to complete the registration process by mail or fax. This Registration form is available in PDF format - you will need the Adobe Reader to view, complete and print the document. The Reader is available free from the Adobe web site. Payment must accompany the registration form and can be made by check or credit card.

Financial Support:

A limited number of scholarships and financial assistance for students and parents are available through the Statewide Independent Living Council of Illinois; call 217-744-7777 for more information.

Scholarships are also available through the Arc of Illinois Consumer Involvement Program; call 815-464-1832 for information about the Arc of Illinois program or visit their website at www.thearcofil.org.

Student participation is so very important for successful transition planning. Funding to support student participation has always been a struggle. Exhibitor Registration:

Do you want to reach over 800 participants who are interested in helping create active, productive lives for young people with disabilities? A limited number of exhibit spaces are available to allow you to reach students, parents, teachers, social service and healthcare professionals.

The 2009 Illinois statewide Transition Conference will have general exhibits open on Sunday 11/8/09 from 1:00 pm to 4:00 pm, on Monday 11/9/09 from 8:00 am to 4:00 pm and on Tuesday 11/10/09 from 8:00 am to 12:00 pm. Healthcare and College Exhibit spaces will be available Monday 11/9/09 from 8:00 to 3:30 pm. Complete the 2009 Exhibitor Prospectus to become an exhibitor, sponsor, or advertiser.

 

Continuing Education Units

Refer to additional information and requirements for the particular continuing education credit in the Conference Program book and at the continuing education informational site at the conference. If you need more information prior to the conference, please e-mail:
transition@cccmeetings.com.

  • Educators Continuing Education Units (CEU), Continuing Professional Development Units (CPDU) will be available upon request.
  • Nursing contact hours for participation in the health care track and conference plenary sessions only has been submitted to the Illinois Nurses Association Approver Unit. Please call Darcy Contri at University of Illinois at Chicago, the Division of Specialized Care for Children, for more information about the nurse contact hours, (217) 558-2350.
  • Social Worker continuing education (CE) and Physical Therapy continuing education credit will also be available.
  • Rehab Counselors Certified Rehabilitation Counselors (CRC) will be available upon request.
  • Physicians, please call Darcy Contri at the University of Illinois at Chicago, Division of Specialized Care for Children, for more information about credits (217) 558-2350.
  • Speech and Language Pathologist and Audiologist will earn a continuing education hour of credit for each hour of attendance provided through Illinois State Board of Education (ISBE). Credit may additionally be given in one-half hour increments. Please call Melanie Fleenor with ISBE at 217-782-5589 for more information.
